Is there an ideal antiarrhythmic drug? A review--with particular reference to class I antiarrhythmic agents.

چکیده
The number of antiarrhythmic drugs has grown dramatically over the last 15 years, because ofthe need for effective and safe agents to prevent or control cardiac arrhythmias. In the clinical situation, logical choice of a particular antiarrhythmic drug depends not only on a demonstration of its effectiveness against various sorts of arrhythmias, but also on a knowledge of its pharmacokinetics, haemodynamics and adverse effects.

Sudden cardiac death is a significant cause of mortality in the United States, with an estimated 400,000 cases per year (1 1). Discovery of an effective antiarrhythmic drug capable of reducing the incidence of sudden cardiac death by as little as 10% would result in the saving of approximately 40,000 lives annually. Obviously, there is a need for an effective form of therapy capable of safely t...

Most antiarrhythmic drugs are potent compounds with a relatively narrow therapeutic index. When prescribed judiciously, they can have a key role in enhancing or prolonging the lives of patients with most common arrhythmias. But when misprescribed, through selection of an inappropriate drug or dosage regimen, the end result may range from inadequate control of the arrhythmia to a proarrhythmic e...

The Sicilian Gambit: an opening move that loses the game? The Sicilian Gambit is the title of a review article on antiarrhythmic drug therapy published simultaneously in the European Heart Journal and Circulation in 1991 1 ' 1. The rather peculiar name is a reference to the Queen's Gambit (an opening move in chess) and the fact that the article, considered by the authors as an 'opening move' in...

Survivors of acute myocardial infarction (MI) remain at risk of sudden cardiac death after being discharged from the hospital. In these patients, frequent premature ventricular contractions (PVCs) and depressed ventricular function herald sudden cardiac death.1-3 These observations triggered the idea that if PVCs were suppressed by antiarrhythmic agents, then sudden cardiac death could be avert...
متن کاملKinetics of use-dependent ventricular conduction slowing by antiarrhythmic drugs in humans.
BACKGROUND Rate-dependent conduction slowing by class I antiarrhythmic agents has clinically important consequences. Class I drugs are known to produce use-dependent sodium channel blockade. If rate-dependent conduction slowing by class I agents is due to sodium channel blocking actions, the kinetics of conduction slowing should be similar to those of depression of sodium current indexes in vit...
